Disease Management Program for Diabetes

StayWell is proud to offer our Disease Management Program for Diabetes—a component of our EnjoyLife Health Management Program. The Disease Management Program for Diabetes is designed to identify members with type 1, type 2 and gestational diabetes. Following the American Diabetes Association Standards of Medical Care in Diabetes, the program will assist in educating these members and emphasize the importance of proper self-management of their chronic condition and regular care. The program will focus on providing support tools to reduce health care service use and costs associated with avoidable complications, such as emergency room visits and hospitalizations. The components of the program include; condition monitoring and reporting, patient adherence, consideration of other health conditions and lifestyle issues, stratified intervention strategies, and access to Diabetes Self-management Training (DSMT). To ensure and improve program efficacy, outcomes are measured and assessed through a collaboration of program staff and partners.

StayWell Diabetes Self-Management Education & Support Program (DSMES)

StayWell is pleased to offer our Diabetes Self-Management Education and Support (DSMES) Program, a component of the StayWell Disease Management Program for Diabetes Mellitus.

This program aims to educate StayWell members with diabetes on effective self-management to prevent complications. Eligible members identified through our Disease Management Program are encouraged to enroll and will receive tools, resources, and support tailored to their needs. The curriculum, based on culturally and linguistically relevant materials and ADCES 7 Self-Care Behaviors, helps members manage their condition and reduce risks.

Sessions, led by certified Pacific Diabetes Care and Education Specialists and supported by community partners, will be available in group settings or one-on-one, as needed. Progress will be monitored to assess the program’s effectiveness.

The DSMES Program, through collaboration with StayWell staff, wellness partners, and providers, aims to improve members’ HbA1c, blood pressure, cholesterol, medication adherence, lifestyle behaviors, and reduce diabetes-related complications and healthcare costs.

StayWell is proud to offer our the Disease Management Program for Coronary Artery Disease (CAD)—a component of our EnjoyLife Health Management Program. The Disease Management Program for CAD is designed to identify members with CAD. Following the American College of Cardiology and the American Heart Association Practice Guidelines, the program will assist in educating members about CAD by emphasizing the importance of proper self-management of their chronic condition and regular care. The program will focus on providing support tools and resources to reduce health care service use and costs associated with avoidable complications, such as emergency room visits and hospitalizations. The components of the program include; condition monitoring and reporting, patient adherence, consideration of other health conditions and lifestyle issues, and stratified intervention strategies.
