Notice of Change in Formulary Status - LifeScan Products

Effective January 1, 2026, Lifescan blood glucose monitors and supplies will be removed from StayWell’s Drug Formulary. This means these products will no longer be covered under your prescription benefit and will be denied at the pharmacy.

Before your current test strips run out, please talk to your healthcare provider about switching to a covered alternative. StayWell’s covered options include:

  • Abbott (FreeStyle Freedom, FreeStyle Lite, Precision)

  • Ascensia (Contour, Contour Next, Contour Plus)

Both brands are reliable and designed to support accurate testing—helping you continue to manage your health with confidence.

To switch, please request your provider to send a new prescription to the pharmacy. A free glucose meter is available from the manufacturer, and your pharmacy can process the claim using the information below:
