Yes. Over-the-Counter OTC COVID-19 tests that are approved by the U.S. Food and Drug Administration (FDA) and purchased on or after January 15, 2022 will be covered by StayWell until the end of the federal public health emergency.

Only tests that are purchased by a member for personal use to diagnose a possible COVID-19 infection will be reimbursed.

These tests don’t require an order from a physician to qualify for reimbursement; however, StayWell will not reimburse for tests purchased for employment or return-to-work programs, travel, or quarantine clearance. StayWell will not reimburse for tests that have been or will be reimbursed from another source or that will be placed for resale.

You can purchase OTC COVID-19 tests that are approved by the FDA at pharmacies, retail locations, and online.

Please note that if you have a residential address registered with USPS, you can easily order 2 sets of 4 free OTC COVID-19 tests per household directly from the federal government. There’s no cost for shipping and you don’t need a credit card.

At this time, yes. Please continue to check our website FAQs for updates on information on our point-of-sale (POS) direct coverage program that will allow you to pick up your test without an out-of-pocket cost from our network of participating locations.

StayWell Health Insurance will reimburse a maximum of 8 tests per member every 30 days. Please note that some test kits or boxes include more than one individual test. StayWell will cover each individual test up to 8 individual tests. For example, if a member buys 5 test kits that include 2 individual tests, StayWell will only reimburse up to 4 test kits/8 individual tests

Please note that each member will not be reimbursed for any tests beyond the maximum of 8 individual tests every 30 days. The lookback period is a rolling 30-days.

If a ninth at-home over-the-counter COVID-19 test is purchased within the lookback period of 30 days and submitted for reimbursement, the ninth claim for reimbursement will be denied.

You will be reimbursed the amount of the cost of each OTC COVID-19 test purchased up to a maximum of 8 every 30 days. For example, if you buy a two-pack of tests for $24, StayWell will reimburse $24. With this reimbursement, 2 of your 8 maximum tests allowed every 30 days will be accounted for.

Simply fill out and sign the StayWell OTC COVID-19 Test Claims Submission Form for each member requesting reimbursement and submit it to StayWell’s Customer Care Department along with the UPC code(s) of the test(s) purchased and your purchase receipt (and delivery receipt if an online order) showing date of purchase and purchase amount.

You can easily submit your signed StayWell OTC COVID-19 Test Claims Submission Form and required documents to StayWell by:

If the claim submission is approved having met all required criteria for reimbursement and is not pending any information or documents, the reimbursement should be mailed within 45 days of receipt of the filled out and signed OTCCOVID-19 Test Claims Submission Form including UPC code(s) of the test(s) purchased and purchase receipt (and delivery receipt if an online order) showing date of purchase and purchase amount.
